Please complete the Pet Information Sheets 1 & 2 in this packet, using a pen.
This Packet Includes:
. Rate Summary
. Legal Considerations (Required at initial interview)
. Veterinary Instructions & Release (Required at initial interview)
. Pet Information Sheets-(please complete Pet Information Sheets 1 & 2 for
each pet, litter, horse, or fish tank etc).
. Service Request (Filled out by pet sitter at initial interview).
Items Covered on Initial Interview:
1. Your signed documents.
2. A key. We will provide a key tag and a code (no name will be put on your key).
3. A alarm system code and phone number, also a code word.
4. Emergency contact information for yourself, and at least 1 other contact.
5. A list, if items you plan to leave out during pet sitter visits
(such as paper towels, medicines, etc).
6. Veterinary contacts & medical information (allergies, conditions).
7. Trip information, including Hotel and if you plan to have any visitors at the house while away.
Keys:
By default, your key will be kept by My "Pet Pals" home pet sitters after service for any future service.
If you would like your keys returned after each service, a flat $10.00 for time and mileage will be charged.
House keys are only returned in person, if requested, not left on the counter or under the mat, only for your protection, unless you insist. If you want the key left, then please leave instructions on how to secure the home and where to place the key. We are not liable for the additional safety risk to your home if you ask us to leave your key in an unsecured manner or location. We take your pet and home safety, very seriously.
